APPOINTMENTS – OFFICE HOURS
Our office is open from 9:00 a.m. to 4:30 p.m. daily, Monday through Thursday, and 9:00 am to noon on Friday. The office is normally open through the lunch hour. Each doctor has scheduled office hours during the week as well as scheduled surgery time. Because they are surgeons, they may be called to the hospital at any time, and your appointment may have to be rescheduled. However, we will do our best to reschedule it in a timely manner. If you are unable to keep your appointment, please call promptly so we can make that time available for other patients.
FINANCIAL POLICIES
It is our policy that the charge for an office visit is paid at the time of your appointment. We accept cash, checks, and most credit cards. If we are contracted with you insurance company, we will bill your insurance company and you will be asked to pay your co-payment at the time of your visit or surgery. We accept assignment for Medicare and will bill them and a supplemental insurance for you at no charge. Please feel free to talk with our Billing Manager to determine what your financial obligation will be and/or to arrange for a payment plan. We will be happy to answer any questions you may have regarding your bill at any time. The billing and insurance number is (530) 244-6951.
Please contact your insurance company to determine if we are in your provider network. Failure to do so may result in additional cost to you. Please call you insurance company if you have any questions about your policy.
DISABILITY FORMS
We will be happy to complete your state disability insurance forms for you at no charge. However, there will be a $12 fee for each additional form that is completed. This fee will be paid prior to the form being completed.
PRESCRIPTIONS
When you need your prescription refilled, call your pharmacy and they will call or fax our office for approval. Because the doctor may not be immediately available to approve a prescription refill request, please allow 24 – 48 hours for his decision. Refills are not available after hours or on weekends except in an emergency.
NOTICE OF FINANCIAL INTEREST
Physicians must inform patients of any financial interests they have in hospitals or outpatient surgery facilities. One or more of our surgeons own an individual interest in Mercy Surgery Center, L.P., Court Street Surgery Center and / or Redding Surgery Center.
